This form is for requesting an update/correction to Player's School Name and School Grade. An email will be sent to administrator and we will update your data. If you do not know School Name, but you know the grade, you may send the request anyway. You do not need to request to upgrade the grade every year because we will add 1 for all scholastic players every year.
